Complete the below form for any announcement requests or budgetary needs for upcoming events. The event’s purpose and details are required in order for your request to be processed for approval. Also indicate if you need to use the church facility or equipment for your activities. You must submit this form no later than 30 days prior to the event. Once your event has been approved, you will be notified by the TWC staff.

For recurring events

Please itemize (example: 2nd Tuesday of each month or if in a series – 12-01, 02, 11, 12, 13)

Such as Media (presentation manager/screen projectors), Sound, Security and Emergency Response, Children's (childcare), Ushers, Worship Ministry, etc.

List any facility needs for your event. Such as tables, tablecloths, kitchen use, room arrangements, etc.
